The undersigned hereby certify that pursuant to the provisions of Massachusetts General Laws Chapter 167, Section 47, as amended, we have disclosed to the Mortgagee that:

   the residential dwelling(s) on the Property are insulated with urea formaldehyde foam insulation ("UFFI") with a formaldehyde gas level of _____ parts per million (PPM), as is indicated in the attached notice required pursuant to Massachusetts General Laws Chapter 255, Section 121, including the location of any UFFI in the dwelling(s) and, if known, when the UFFI was installed, and the attached copy of the test results for formaldehyde gas on the Property.

   there is no urea formaldehyde foam ("UFFI") in any rediential dwelling(s) on the Property

The undersigned acknowledge that the Mortgagee is relaying upon this certification in granting the mortgage loan secured by the above-referenced Property, and in consideration of the granting of said loan, the undersigned agree, jointly and severally, to indemnify and hold harmless the Mortgagee and its successors and assigns from and against all claims, liability, damage, loss or injury, including all reasonable costs, expenses and attorney´s fees, arising from the failure of either or both of the undersigned to comply with the requirements of said Section 47.
